What to Bring
- Government-issued photo ID
- Insurance card (for superbill reimbursement)
- List of current medications (name, dose, frequency)
- Records from prior psychiatric treatment (if available)
- Any prior psychological or neuropsychological testing
Payment Information
- Out-of-network provider — payment due at time of service
- Credit/debit card, check, or cash accepted
- Superbill provided for PPO insurance reimbursement
- No membership or annual fees of any kind
