Overview
Methylphenidate (Ritalin, Concerta, Focalin, Daytrana) was the first ADHD medication and has the longest safety record of any stimulant. It works by a different mechanism than amphetamines and is preferred by some patients who experience side effects with amphetamine-based medications. The extensive range of formulations provides dosing flexibility for different daily schedules.
How It Works
Methylphenidate primarily blocks the reuptake of dopamine and norepinephrine without significantly increasing their release (unlike amphetamines, which do both). This "reuptake blockade" mechanism produces effective symptom control with a slightly different side effect profile than amphetamines.
Available Formulations
- Ritalin IR (5mg, 10mg, 20mg) — 4-hour duration
- Ritalin LA (20mg, 30mg, 40mg) — 8-hour extended release
- Concerta (18mg, 27mg, 36mg, 54mg) — OROS formulation, 10–12 hour duration
- Focalin / Focalin XR (dexmethylphenidate) — d-isomer only, more potent per mg
- Daytrana patch — transdermal, useful when oral medications cause appetite suppression

Monitoring: Blood pressure, heart rate, weight, sleep. Some patients who experience anxiety or mood effects with amphetamines tolerate methylphenidate better. Switching between formulations is common to optimize duration and side effect profile.
